Compensation is $35–$52 per hour. The Intake Registered Nurse (RN) role is an onsite, in-office position in Saint Albans, VT, with a Monday–Friday, 8 a.m.–4 p.m. schedule and one weekend per month plus rotating holidays. ; the posting describes a competitive wage and offers up to a $5,000 sign-on bonus. VNA & Hospice of the Southwest Region provides home health and hospice care to people of all ages. The Intake RN screens and processes referrals so clients can receive timely, appropriate home care and hospice services. The role coordinates with physicians, referral sources, scheduling, billing, and quality teams to support compliant, coordinated care and patient goals.

What you'll do

• Screen, triage, and process service requests
• assess referrals against agency policies and admission criteria in collaboration with physicians and referral sources
• use referral and reporting systems
• coordinate clinical services and workflow
• communicate priority visit needs and high-risk situations
• ensure referral data is accurate and complete
• collaborate with scheduling, billing, and quality departments
• and help complete required face-to-face documentation.
