The nurse responds to after-hours patient-care calls, assesses changing needs, makes visits as appropriate, and coordinates care with families, physicians, and the hospice interdisciplinary group. The role supports continuity of hospice care through clinical assessment, care planning, treatment, documentation, and communication.

What you'll do

• Assess patients and families for hospice needs
• respond to triage calls and make visits for changing conditions
• complete weekend admissions
• develop and revise care plans
• administer prescribed medications and treatments
• counsel and educate patients and families
• document care
• communicate with physicians and community health contacts
• coordinate with the interdisciplinary group
• remain available for after-hours patient-care calls and make visits as needed.
