Nurse Liaison

The Nurse Liaison is an onsite role supporting Tempe and Phoenix, AZ, with a Monday–Friday schedule from 8:00 a.m. to 5:00 p.m. Compensation is not specified. Soleo Health provides specialty pharmacy and infusion services in homes and alternate care settings. The role coordinates transitions from hospitals and healthcare facilities to home or alternate-site infusion therapy. It supports safe, timely discharges through clinical assessment, patient and caregiver education, and coordination among hospital teams and Soleo Health departments.

What you'll do

• Collaborate with physicians, case managers, and discharge planners to identify eligible patients
• assess clinical needs and discharge readiness
• coordinate with pharmacy, operations, and reimbursement teams
• educate patients and caregivers on IV therapy, central-line care, safety, and infusion devices
• educate home health staff when needed
• collect clinical documentation
• document interactions and care-plan updates
• serve as a liaison between hospitals and the company
• maintain patient contact during hospitalization and early therapy
• support referral outreach and business development.
