Social Worker Case Manager — Mental Health Clinic

The Social Worker Case Manager role is onsite in Fayetteville, NC, supporting the Mental Health Clinic at Womack Army Medical Center. The schedule is Monday through Friday, with eight-hour shifts totaling 40 hours per week, scheduled between 7:00 a.m. and 6:00 p.m. Compensation is not specified. GiaCare supports military treatment facilities and the patients, families, and communities they serve. The case manager coordinates behavioral healthcare for military service members and other eligible beneficiaries, including people transitioning from inpatient psychiatric care, emergency treatment, or hospitalization after a nonfatal suicide attempt. The role supports continuity of care through risk assessment, safety planning, referrals, multidisciplinary coordination, and follow-up until patients connect with the next level of care.

What you'll do

• Lead mental health case-management operations
• coordinate care and follow-up for patients transitioning from inpatient psychiatric care, emergency-department treatment, or medical hospitalization
• support High Risk Suicide Pathway patients through face-to-face contact, safety monitoring, and care coordination
• conduct suicide-risk assessments, develop and monitor collaborative safety plans, provide crisis intervention, and coordinate inpatient admissions when required
• review documentation and develop individualized care plans with patients, families, providers, and treatment teams
• coordinate referrals and access to behavioral health, medical, transportation, housing, benefits, and community resources
• advocate through military and community systems when appropriate
• coordinate and chair multidisciplinary meetings
• collaborate with RN Case Managers
• provide clinical supervision and licensed counseling or psychotherapy
• and complete records, coding, tracking, and reporting.
