Upward Health is an in-home, multidisciplinary medical group providing whole-person care for physical, behavioral, and social health needs. The Care Specialist engages high-complexity patients, supports enrollment, facilitates virtual appointments, and helps patients follow care plans. Through outreach, education, and coordination with providers and families, the role supports improved care adherence and patient outcomes while helping prevent unnecessary hospitalizations.

What you'll do

• Conduct patient outreach and support enrollment

• use phone calls, home visits, and community outreach to engage patients
• maintain accurate patient documentation in electronic health record systems.
