What you'll do

• Conduct initial and ongoing assessments
• develop, implement, and monitor individualized family service plans with families and multidisciplinary teams
• coordinate and schedule services with therapists, educators, healthcare providers, and community resources
• provide information, support, and advocacy to families
• maintain documentation and regulatory compliance
• facilitate communication among stakeholders
• participate in team meetings, training, and professional development.

Why this role
