What you'll do

• Provide skilled nursing care
• monitor and assess the patient’s condition
• execute the care plan and communicate with the RN Case Manager
• administer prescribed medications and treatments
• provide tracheostomy, ventilator, and enteral-feeding support
• assist with personal care, mobility, meals, and light housekeeping
• accompany the patient to approved appointments and outings as authorized
• report changes
• document care using the approved Electronic Visit Verification system.

Why this role

Requires graduation from an accredited licensed nursing program and a current Pennsylvania nursing license. Experience with tracheostomy care, PEG tubes, and ventilators is strongly preferred.
