Job overview
The IBHS Care Coordinator role is onsite in Philadelphia, PA, with a Monday–Friday schedule from 8:30 AM to 5:00 PM. Pay is $25–$30 per hour. The position supports Dunbar Wellness Group’s Intensive Behavioral Health Services (IBHS) program, which provides community-based behavioral health services. The coordinator connects clients and families with clinical, administrative, and agency teams while supporting access to authorized services and continuity of care. The role monitors service delivery, coordinates staffing and intake, maintains accurate records, and assists with quality assurance and compliance activities.
What you'll do
- Coordinate assigned IBHS cases and prioritize client needs
- monitor authorized services and address potential care gaps
- communicate with clients and families about services, scheduling, staffing, and program needs
- conduct routine quality review calls
- advocate for clients and help resolve service concerns
- coordinate case staffing and monitor schedules, coverage, productivity, and caseloads
- maintain tracking systems and client records
- coordinate intake and service initiation
- review documentation and conduct chart reviews
- assist with quality assurance and compliance audits
- connect clients with community resources
- participate in meetings and supervision
- conduct weekly site visits as assigned
- provide occasional direct support in school or community settings when necessary and within qualifications and assigned responsibilities.
