The RN manages patient care plans and coordinates with LPNs, therapists, and physicians, combining home visits, assessments, education, and documentation. The role supports patient outcomes, recovery, and independence at home through clinical judgment and coordinated care.

What you'll do

• Develop and adjust care plans with the care team
• complete patient visits, OASIS assessments, and documentation
• coordinate care with LPNs, therapists, and physicians
• educate patients and families about diagnoses, medications, and home safety
• advocate for patients
• and help drive outcomes.
