What you'll do

• Manage a panel of patients
• conduct psychosocial assessments and develop, implement, and update care plans
• coordinate with caregivers and support systems
• identify psychosocial risks and service needs
• advocate, refer, coordinate, and follow up on support
• provide individual counseling as needed
• facilitate family conferences and support groups
• engage community services
• coordinate discharge planning
• participate in caregiver workshops and educational sessions
• may review assessments completed by other social work staff
• document accurately and participate in team care planning.
