What you'll do

• Assess patient condition and initiate start-of-care face-to-face visits
• take vital signs and review medications
• administer medication as prescribed
• dress or redress wounds
• provide patient education
• coordinate with the interdisciplinary team on care-plan progress
• provide supervision and competency checks for RNs, LPNs, and CNA/HHAs as needed
• recommend devices, tools, or medications that may improve quality of life.
