The Licensed Practical Nurse (LPN) / Licensed Vocational Nurse (LVN) is an onsite role based in San Luis Obispo, CA. This role exists to monitor health conditions, administer medications, and provide prescribed medical treatments to individuals with diverse needs. The nurse acts as an advocate and liaison to ensure consistent, high-quality care and supports the overall independence and well-being of the individuals served.

What you'll do

• Monitor health conditions, administer medications, provide prescribed medical treatments, conduct assessments, implement Individual Service Plans (ISP), provide staff training on infection control and medication administration, and serve as a liaison with medical providers.
