Job overview

The Qualified Intellectual Disabilities Professional (QIDP) Case Manager role is onsite in Park Forest, IL. Compensation is $57,000 per year. What you'll do

  • Develop and implement individualized programs and plans
  • schedule and monitor member activities
  • complete assessments and required reports
  • coordinate professional and consultative services, medical appointments, and follow-up care
  • maintain accurate member records and outcome documentation
  • communicate with guardians, healthcare providers, and community agencies
  • participate in person-centered planning and committee meetings
  • coordinate discharge or transfer planning
  • and assume periodic on-call responsibilities and respond to emergencies as needed.
