Independent Living Skills Trainer

The Independent Living Skills Trainer is an onsite role serving clients throughout Santa Clara County, CA. Pay is $23.06 per hour. Hope Services provides services to people with developmental disabilities and mental health needs. The trainer assesses client needs and provides individualized instruction and follow-up in homes and community settings. The role supports clients’ community integration and progress toward goals through training, advocacy, documentation, and coordination with families and care providers.

What you'll do

• Provide one-to-one assessment, training, and follow-up in clients’ homes and communities
• provide travel destination training
• monitor assigned clients and maintain required records and reports
• assess and document progress
• participate in case reviews and goal planning
• coordinate transportation
• advocate for clients and provide intervention or consultation as needed
• act as a mandated abuse reporter.
