What you'll do
- Assess and reassess hospice patients’ nursing needs in their homes
- provide dietary counseling, nursing services, treatments, and preventive procedures
- observe and report changes in physical or emotional condition
- teach and counsel patients and families
- supervise and train nursing personnel
- develop and reevaluate care plans with the interdisciplinary group
- maintain records and follow hospice policies and HIPAA requirements
- collaborate with the after-hours team.
