What you'll do

• Manage cases and conduct starts of care, admissions, recertifications, resumptions of care, transitional care, clinical reviews, and other assessments and services
• perform occasional skilled nursing visits
• document and communicate patient status and changes to the care team
• coordinate with physicians, agency staff, administrators, caregivers, clinicians, clients, and families
• supervise nursing and aide staff
• support orientations, training, in-services, and supervisory reviews.
