The role assesses clients, develops individualized care plans, supports and trains caregivers, and documents care while ensuring compliance with applicable requirements.

What you'll do

• Review caregiver applications and interview candidates
• train and orient caregivers
• administer medications outside CMT scope, such as B12 injections
• onboard and manage high-needs clients
• assess clients for home care and develop individualized care plans
• supervise and support caregivers
• participate in case conferences
• submit clinical and progress notes
• ensure compliance with federal, state, and accreditation requirements.

Why this role

PRN/as-needed hourly W-2 position with no guaranteed weekly hours; schedule coordinated in advance with the Administrator. OASIS experience and prior supervisory experience are desirable; infusion experience is preferred.
