What you'll do

• Provide direct patient care, treatment assessment, and medication management
• evaluate patients and document pharmacy recommendations, plans, interventions, and medication reconciliation
• review drug indications, dosing, allergies, interactions, laboratory monitoring, and duplication
• monitor drug utilization and therapy outcomes
• provide medication education
• respond to Level I trauma, code blue, and massive blood transfusion emergencies
• support antibiotic stewardship, pharmacokinetic dosing, renal and hepatic dosing, patient education, and discharge planning.
