What you'll do

• Provide case management and skilled nursing visits in patients’ homes
• develop treatment plans based on physician orders and initial assessments

• coordinate discharges with the physician and clinical manager
• provide nursing care and written care plans
• monitor patient and family compliance
• complete clinical documentation
• and identify and use appropriate resources and referrals.
