What you'll do

• Perform wound and skin assessments
• develop, implement, and monitor wound care plans
• provide wound treatments and preventive skin care
• monitor wound progression and communicate changes
• coordinate with the interdisciplinary team on wound healing
• document assessments and treatments
• assist with resident admissions, nursing assessments, record review, care-plan implementation, resident and family education, and transition coordination.
