The Community Health Worker — Community Health Navigation role is an onsite, full-time position in New York, NY, with work split between NewYork-Presbyterian Brooklyn Methodist Hospital and CAMBA. What you'll do
- Maintain professional relationships and confidentiality
- participate in outreach and community events
- attend case management and interdisciplinary meetings
- accompany patients to medical and social service visits as needed
- visit participant homes for education and support
- conduct outreach and schedule intake appointments
- complete needs assessments
- help clients advocate for themselves and access benefits
- document interactions and progress
- provide health education on blood pressure, healthy lifestyle changes, and medication management
- assist with telehealth visits and medication reconciliation as needed.
