What you'll do

• Provide assigned personal care, including bathing, oral hygiene, dressing, and feeding assistance
• observe skin condition and report pain, symptoms, or changes to the RN case manager
• assist with ambulation, exercise, range of motion, transfers, and positioning as directed
• provide reminders for self-administered medications as allowed
• assist with light laundry and homemaking
• document care and follow care procedures.
