What you'll do

• Manage admissions from inquiry and referral through transition
• respond to hospital, community, and family inquiries
• conduct tours and pre-admission assessments
• coordinate with nursing, business office, and interdisciplinary departments
• complete admission documentation and payer verification
• monitor referral activity and census data
• develop social-service programs
• complete social histories and assessments
• provide counseling and education
• coordinate community services
• contribute to care planning and discharge planning
• protect residents’ rights and confidential health information.
