The Registered Nurse (RN), Home Hospital Program role is onsite in Jacksonville, FL. Pay is $45–$50 per hour. Concierge Home Care’s home hospital division provides higher-acuity skilled nursing care in patients’ homes. The nurse assesses patients, develops and adjusts care plans, provides skilled treatments and education, and coordinates with physicians and interdisciplinary teams to support recovery and safe healing at home.

What you'll do

• Conduct patient assessments and develop care plans
• provide advanced skilled nursing treatments and patient education
• coordinate care with physicians and interdisciplinary teams
• monitor patient progress and adjust care plans
• document visits using AI-assisted tools. Common treatments may include TPN administration, chest tube management, lab draws, IV therapy, wound care, post-surgical recovery support, and respiratory care.
