The Intake and Evaluation Screener (I&E Screener) role is in person in Belle Glade, FL. South County Mental Health Center provides behavioral healthcare services in Southeast Palm Beach County. The screener coordinates diversion, linkage, and care transitions for people involved in criminal justice and behavioral health systems. The role helps identify appropriate community care options, coordinate with courts and partner agencies, and maintain documentation that supports continuity of care and regulatory compliance.

What you'll do

• Assist with diversion to appropriate community settings
• evaluate client needs and identify alternatives to hospitalization when clinically appropriate
• coordinate community resources and treatment options
• support transitions from state facilities to community care
• coordinate with prosecutors, public defenders, courts, hospitals, and agencies
• gather case and treatment information
• complete referrals and transitional service authorizations
• maintain client records, correspondence, and reports.
