What you'll do

• Provide behavioral health and substance use assessments

• conduct substance use screenings and brief counseling or referrals
• develop treatment plans and maintain electronic records
• collaborate with the primary care team
• refer patients to community resources or specialists
• manage crises as needed
• monitor medication use and encourage coordination with prescribers
• support workshops and integrated care collaboration.
