The Family Care Coordinator / Case Manager (FFPRS) role is based in Ashland, KY, with a salary range of $44,000–$48,000 per year. KVC Kentucky provides in-home behavioral healthcare, family preservation and reunification, foster care, and youth substance abuse treatment. The coordinator works with children, families, community partners, and providers to assess safety, develop and implement family action plans, connect families with resources, and support child well-being and family preservation. The role also maintains case documentation and communication with referring DCBS workers.

What you'll do

  • Complete face-to-face family intake within 24 hours
  • assess child safety and develop safety plans
  • serve as a point of contact for families and DCBS
  • gather assessment information
  • develop family agreements and action plans
  • coordinate providers and services
  • conduct team meetings and family contacts
  • provide direct services to reduce risks and improve child well-being
  • connect families with community resources
  • document services and progress
  • participate in case consultation and aftercare
  • travel for home visits and partner contacts
  • participate in an on-call rotation.
