Behavioral Health Substance Use Disorder Case Manager

What you'll do

• Assess behavioral health, recovery, social, and community-support needs
• develop and maintain individualized case management plans
• monitor progress
• coordinate internal and external services
• connect clients with community resources
• help address barriers involving transportation, housing, employment, medical care, benefits, and legal concerns when applicable
• advocate for clients
• participate in treatment meetings
• coordinate discharge and aftercare
• follow up on referrals
• maintain timely, accurate records and confidentiality
• attend supervision, meetings, and training.
