Start of Care Specialist

What you'll do

• Participate in new Starts of Care
• work alongside and coach Care Professionals
• help them understand client routines, preferences, needs, home environments, and Plans of Care
• observe care and identify gaps or safety concerns
• partner with Client Care, Scheduling, Care Pro Success, and Operations to resolve barriers
• document observations, actions, and next steps
• support new clients as they work toward stable care.
