What you'll do

• Coordinate weekend patient care and oversee field clinicians
• schedule and complete patient visits
• process calls and referrals
• assess patients and families
• develop and manage plans of care
• coordinate medications, equipment, supplies, and services
• administer prescribed treatments
• supervise hospice aides and LPNs as indicated
• educate and support patients and families
• document care and communicate with physicians and the interdisciplinary team.
