What you'll do

• Receive, enter, and route referrals
• contact members to schedule assessments, appointments, and follow-up calls
• organize work queues and track tasks
• request medical records and discharge summaries
• coordinate appointments, transportation, and community resources as directed
• schedule interdisciplinary care meetings
• prepare correspondence and educational materials
• maintain documentation and assist with reports and audit preparation
• refer clinical questions and urgent concerns to licensed staff.
