Job overview
The Field RN Case Manager is a full-time onsite position based in Washington, DC. The pay rate is $162,801.00 per hour. This role supports MedStar Family Choice D.C. by conducting comprehensive clinical assessments and coordinating care for enrollees. This role exists to serve as the central point of contact for case management, integrating physical, behavioral, and social health into a person-centered system of care. The Case Manager conducts field-based assessments, coordinates services, and ensures members receive appropriate care in the least restrictive setting.
What you'll do
- Conduct annual and periodic comprehensive clinical assessments.
- Serve as the central point of contact for case management.
- Refer enrollees to programs such as maternal health, behavioral health, and food insecurity relief.
- Coordinate care with providers and community resources.
- Prepare reports on work progress and resolve backlogs.
