What you'll do

• Conduct outreach, engagement, and follow-up with members
• coordinate services and appointments, including reminders and transportation
• support health-promotion activities
• share member circumstances with the care team
• track referrals and help members access community resources and social supports
• participate in case conferences
• and assist care managers in identifying unmet health-related resource needs. The role may require transporting clients to organization-sponsored services or resources.
