What you'll do

• Manage an individual caseload of single adult participants
• provide supportive counseling and addiction treatment services
• participate in treatment planning and goal setting
• connect participants with community resources and life-skills support
• document progress and treatment plans
• participate in clinical team meetings
• coordinate with collateral agencies
• provide crisis intervention and stabilization as needed.
