What you'll do

• Develop and maintain relationships with physicians, hospitals, skilled nursing facilities, assisted living communities, and other referral partners
• educate partners about hospice eligibility, services, and benefits
• generate appropriate referrals through outreach and follow-up
• collaborate with admissions, clinical teams, and leadership
• develop territory plans and outreach strategies
• document activities and territory updates
• represent hospice at community events and educational programs
• respond to referral opportunities and concerns
• prepare presentations and develop key account plans.
