Hospice Music Therapist

What you'll do

• Provide music therapy under individualized plans of care
• assess and evaluate therapeutic status and contribute to care plans
• direct treatment and identify priority needs
• report patient reactions or condition changes to physicians
• instruct personnel and caregivers on relevant therapy goals
• prepare clinical and progress summaries
• provide family and caregiver consultation
• participate in case conferences, interdisciplinary work, quality improvement, and hospice team orientation.
