What you'll do

• Serve as liaison among hospital teams, patients, and clinicians
• conduct bedside assessments
• coordinate patient-centered transition plans with case managers and physicians
• manage referrals and admissions
• build relationships with hospital partners
• complete post-discharge follow-up within 48 hours
• coordinate primary care and collaborate on readmission reduction and outcome improvement.
