What you'll do

• Work with the Patient Clinical Manager and Hospice Service Coordinators
• support patients, families, and clinicians
• schedule staff visits according to patient care plans as needed
• perform chart audits
• use reports and EMR features to monitor clinical, quality, and financial metrics
• monitor field staff caseloads and scheduling against regulatory timepoints
• follow up with patients, families, and facilities as directed
• attend mandatory in-services and maintain professional knowledge
• comply with HopeHealth policies and quality, safety, infection control, and HIPAA requirements.
