What you'll do

• Screen potential participants for eligibility
• gather treatment history and historical hospitalization and incarceration data
• facilitate assessments and recommend levels of care
• work with the treatment team on individualized plans
• compile and communicate compliance and progress information
• attend and facilitate weekly staffing and court hearings
• coordinate team meetings and community resources
• provide individual and group interventions
• monitor services and maintain accurate, confidential clinical records.
