The Part-Time Registered Nurse — Intensive Community Wrap-Around Services role is onsite in Chelsea, MA, at $38.46 per hour, for 20 hours per week. The nurse joins North Suffolk’s Intensive Community Wrap-Around Services Team, supporting people served by the Community Treatment Team. The nurse contributes clinical assessment, monitoring, consultation, direct interventions, and care coordination for people with mental illness and related needs. The role guides health-related team interventions and supports wellness, recovery, and self-management.

What you'll do

• Participate in assessment, treatment planning, and service delivery
• monitor medical and psychiatric status
• provide consultation, direct interventions, and care coordination
• collaborate on comprehensive assessments
• support health and wellness training
• provide outreach to people who are difficult to engage
• oversee the MAP process and may administer medication that MAP-certified staff cannot administer.
