The Registered Nurse — Home Health role is based in Marion, KS, and pays $34.85–$37.31 per hour. The nurse provides care in clients’ homes and coordinates services for clients of all ages. The role supports clients’ wellness and independence through skilled nursing, individualized care planning, patient and caregiver education, case management, and supervision of LPNs and Home Health Aides.

What you'll do

• Coordinate patient assignments, referrals, and services
• assess client needs and update individualized care plans
• provide direct skilled nursing and teach care skills to clients and caregivers
• document care in accordance with agency and payer requirements
• conduct supervisory visits for Home Health Aides and LPNs
• communicate clinical changes to physicians and involved personnel
• participate in quality management and required training
• work the on-call schedule as required.
