Case Manager – Developmental Disabilities Waivers

Job overview

The Case Manager role is onsite in Minneapolis, MN, with a Monday–Friday schedule during standard business hours and the possibility of fully remote work after 3–6 months of onboarding. Starting pay is $50,000–$52,000 per year, depending on qualifications, plus a $1,500 hiring bonus for external candidates. This role advocates for clients, connects them with community resources and services, and provides client-centered case management. The work supports clients’ access to coordinated services and helps them pursue greater independence and well-being.

What you'll do

• Advocate for Fraser clients
• connect clients to community resources and services
• provide client-centered case management to people receiving CADI/BI waiver services or DD services
• maintain documentation and case notes
• meet with clients two to three times per year. Travel to client meetings is required, which may occur two to three times per week.
