What you'll do

• Maintain a caseload and provide goal-directed mental health skill-building services
• support functional skills, activities of daily living, community-resource access, medication management, and monitoring of health and nutrition
• provide supportive counseling and crisis intervention as needed
• develop and implement individualized service plans and discharge plans
• prepare monthly and quarterly reports
• document services
• coordinate care and communicate with program staff and treatment providers
• transport clients to appointments.
