The role coordinates behavioral strategies with medical treatment and works with primary care, specialty providers, families, and caregivers to support patient-centered care.

What you'll do

• Manage a caseload
• conduct patient visits at a cadence tailored to patient needs
• observe and document behavioral patterns
• implement structured behavioral protocols
• monitor changes in behavior, function, and cognition
• collaborate with providers and participate in case reviews and telehealth consults
• educate and coach caregivers
• and participate in or lead education initiatives for families and the community.
