What you'll do

• Represent Legacy Hospice in hospitals and transitional care settings
• build relationships with physicians, case managers, discharge planners, and healthcare partners
• conduct onsite clinical assessments for preliminary hospice eligibility
• educate patients and families about hospice services, benefits, advance directives, and consents
• coordinate referrals, documentation, and care initiation
• maintain Homecare Homebase records
• respond to electronic referrals
• support referral-source satisfaction, community education, quality improvement, and strategy meetings.
