What you'll do

• Provide preventive care and manage chronic and complex conditions
• coordinate care with medical professionals using the Patient Centered Medical Home as a guideline
• build long-term patient relationships and support health self-management
• diagnose and document conditions
• use the Electronic Health Record system
• work with the clinic RN or SDPI coordinator on diabetes prevention and treatment
• participate in quality improvement
• maintain medical records
• and collaborate on policies, procedures, and quality assurance.

Why this role
