What you'll do

• Lead Connect for Life activities using the program curriculum
• plan and host virtual and in-person events
• contact members and conduct one-on-one home visits
• document outreach and attendance
• identify community ambassadors
• enroll prospective members
• and connect members with health plans and care resources. English/Spanish bilingual ability is required. The posting describes travel to member and event locations, including a stated willingness to travel 30–40 miles.
