Apartment Program Case Manager

What you'll do

• Manage a resident caseload and coordinate services
• maintain client and medication records
• oversee medication self-administration and budgeting support
• inspect apartments and address safety concerns
• liaise with landlords
• support daily living skills
• conduct fire drills and check smoke alarms
• coordinate treatment and community referrals
• respond to crises with BHSN Crisis or treatment providers
• transport residents to appointments and errands
• support program transitions.
