What you'll do

• Conduct initial and ongoing patient and family assessments
• contribute to care plans and interdisciplinary team conferences
• document visits, calls, and referrals
• provide counseling and crisis intervention when indicated
• advocate for patients and families
• coordinate referrals with community agencies
• support bereavement work as assigned
• follow hospice policies.
