Nurse Care Manager

The Nurse Care Manager role is remote for residents of Oregon, Washington, and Idaho. Compensation is $30–$37 per hour, depending on experience. Praxis Health is a family of medical groups providing community-based healthcare in Oregon. The nurse coordinates care and follow-up, guides patients and caregivers through the healthcare system, and communicates clinical updates across care teams. The role supports evidence-based care, continuity across settings, and efforts to improve outcomes and reduce avoidable hospital use.

What you'll do

• Serve as patient advocate and navigator
• review clinical documentation and provide condition-specific education
• conduct hospital and emergency-room follow-up calls
• reconcile medications and allergies
• support medication adherence and high-risk patient outreach
• communicate updates to primary care providers
• coordinate referrals and continuity across care settings
• document interactions in the EHR
• participate in quality and population-health initiatives.

Why this role

Remote opportunity is limited to residents of Oregon, Washington, and Idaho. Schedule is Monday–Friday in eight-hour shifts or four nine-hour shifts per week. Current RN licensure in the state of practice is required.
